Ar draws Clwstwr Ysgolion Aberd\u00e2r, cwblhaodd deg ysgol (dwy uwchradd, wyth cynradd) Arolwg Iechyd a Llesiant Myfyrwyr SHRN yn rhan o gylchoedd uwchradd 2023 a chynradd
2024 SHRN. Er bod data SHRN wedi dod o’r deg ysgol hyn, cyfrannodd pob un o’r pymtheg ysgol sy’n rhan o’r Gymuned Ymarfer at nodi’r flaenoriaeth clwstwr a rennir, gan dynnu ar eu cyd-destun lleol, eu profiad a’u tystiolaeth ehangach.<\/p>\n\n\n\n


Yr hyn a ddilynodd oedd prosiect strwythuredig, cydweithredol wedi\u2019i arwain ar y cyd gan D\u00eem Ysgolion Hybu Iechyd a Llesiant Bwrdd Iechyd Prifysgol Cwm Taf Morgannwg (BIPCTM) ac wedi’i lywio gan ddata lefel ysgol SHRN a data\u2019r Bwrdd Iechyd lleol. Gan weithio’n agos gydag ysgolion a phartneriaid ehangach, fe wnaethant sefydlu Cymuned Ymarfer (CY) Iechyd a Llesiant i ddehongli canfyddiadau SHRN, nodi blaenoriaethau a rennir, a chydlynu camau gweithredu ymarferol ar draws y clwstwr. <\/p>\n\n\n\n

<\/p>\n\n\n\n

Mae hefyd yn cynnig model y gall clystyrau eraill ei addasu a’i ailadrodd yn eu cyd-destunau eu hunain. Mae’r astudiaeth achos hon yn nodi’r hyn a ddigwyddodd yn ymarferol, gan ddangos sut roedd ysgolion yn defnyddio data SHRN wrth wneud penderfyniadau pob dydd i lywio gwaith gwella, cryfhau ymarfer ysgol gyfan a chefnogi gweithgareddau sy’n gysylltiedig \u00e2’r dull Ysgol Gyfan, paratoi ar gyfer Estyn a\u2019r Cwricwlwm i Gymru. Mae’n adeiladu ar y bartneriaeth hirsefydlog rhwng SHRN ac Iechyd Cyhoeddus Cymru, sy’n cefnogi ysgolion ledled Cymru i ddefnyddio data iechyd a llesiant o safon i lywio gweithredu lleol.<\/p>\n\n\n\n
